HOW TO RETRIEVE USERNAME/OBTAIN NEW PASSWORD

To retrieve a forgotten username and obtain a new password:

1. Click the "click here" link after the "If you have forgotten your username or password" below the Login button.
2. Enter the required information accurately. 
3. If you wish to clear the fields and re-enter information. Use the "reset" button.
4. When done, click "submit". Your username/new password will be sent to your e-mail and will be available shortly.
